Medical Residential Services

Medical Residential Services: a residential service provided in the individuals residence where direct skilled nursing services and rehabilitative services and support are provided enabling individuals to acquire, retain or improved skills necessary to reside in a community-based setting. The service included direct assistance as needed with activities of daily living (e.g., bathing, dressing, personal hygiene, eating, and meal preparation excluding cost of food), household chores essentials to the health and safety of the individual, attending appointments and interpersonal and social building. Medical Residential services also manage personal funds, manage health care needs, Medication administration, Therapeutic goals and objectives and offer transportation for the individuals served.

Clara Butler NPCAD provides skilled nursing services for individuals requiring more than generalized medication and treatment. These services allow enrollees to live in the community and participate in outing in the community as tolerated. Nursing hours provided vary from case to case based on the individual’s needs and physician orders.

Assisted Living Memory Care

Supported Living Services

SL enables an enrollee to acquire, retain, or improve skills necessary to reside in their own home and/or community-based setting including direct assistance with activities of daily living essential to the health and safety of the enrollee, funds management, attending appointments and interpersonal and social skills building to enable the enrollee to live in a home in the community.  Services shall also include the following:
•    Personal funds Management
•    Therapeutic Goals and Objectives
•    Management of Health Care Needs
•    Medication Administration Assistance
•    Transportation

This policy was developed as an extension of CBNPCAD Supportive Living Transportation policy to address the order in which transportation serves will be conducted for the individuals in our agency that receive this service. These services will afford this agency the opportunity to transport individuals on individual chosen outings,  non-emergency visits to and from approved activities specified in the plan of care, individual’s PCSP, and daily schedules in the individuals’ home.
The purpose of this policy is to provide guidelines on transportation individuals into the community safely and to be utilized to meet the individual’s needs, including appointments, community outings, and other places that the individual desires to explore. CBNPCAD has also established guidelines for the use and safety of personal vehicles in the provision of services to individuals we serve.

Nursing Services

Nursing enables our agency to provide skilled nursing service in the home of the enrollee according to the doctor’s orders. We can provide a service by visits, and by hours ordered by the M.D. Our DSP/PA staff members on duty can attend medical appointments with the individuals and this will free up time for family members to handle other matters knowing they are in good hands.

Assisted Living Short Term Care

PA Services

PA’s are assigned to go to the individuals home in which they live with other family members and our PA/DSP staff worked assigned shifts to assist the individual  with activities of daily living, attend medical appointments with them, help with medication administration reminders, and assist with light housekeeping in areas that is used by the individual; they can wash, iron and fold clothing, and help prepare light meals for the individual.  The family home should provide safety in accordance with the person’s mobility, support, and needs as indicated in their PCSP and in accordance with DDA guidelines/or Care Plan. The criteria for admission consist of qualifying for the services requested, and order, and an approved service plan.

Community Participation

Activities to assist the individual in their goals to stimulate their mind and promote learning/ take our learning growth.
Community Participation will be conducted on weekdays as well as weekends when requested as needed for appointment or outing of the individual’s choice.

Respite Services

Are services provided to an individual when unpaid caregivers are absent of incapacitated due to death, vacationing, hospitalization, illness, or injury, or when unpaid caregivers need relief from routine care giving responsibilities. Respite maybe provided in the individual’s place of residence, in a Family Model Residential Support Home, in a Medicaid Certified ICF/MR, in a home operated by licensed residential provider, or in the home of an approved respite provider. The Respite provider may also accompany the individual on short outing for exercise, recreation, shopping or other purpose while providing respite care.

The individual receiving residential services (i.e., Supported Living, Residential Rehabilitation, Medical Residential Services, and Or Family Residential Support) Shall not be eligible to receive Respite as a service unless it changed and approved by DDA. The cost of room and board shall be excluded from Respite reimbursement if Respite is provided in a private residence. Respite shall be limited to a maximum of 30 days per individual per year. The individual receiving Respite shall be eligible to receive Individual Transportation Services as well.

Supported Employment Day Services

The provider of Supported Employment Day Services reimbursed on a per diem basis must:

1)    The Employment and Community First CHOICES program is administered by Tenn Care through its contracted managed care organizations.  It offers services to people with intellectual and developmental disabilities.  Services in the program will help people become employed and live as independently as possible in the community.

2)    Have a job coach employed by the Day Services provider on site who is supervising the enrollee; or

Individual Transportation Services:

The waiver definition shall apply to all individual transportation services provided in a Medicaid waiver. The waiver definition shall also be used to define individual transportation services provided in other DDA-funded programs. The waiver definition for individual’s transportation services approved by the Center for Medicaid Services (CMS) is:

Individual Transportation Services shall mean non-emergency transport of an enrollee to and from approved activities specified in the plan of care. An individual receiving Orientation and Mobility Training shall be eligible to receive Individual Transportation Services to the extent necessary for participation in Orientation and Mobility Training. Individuals who receive Respite, Behavioral Respite Services or Personal Assistance shall be eligible to receive Individual Transportation Services only to the extent necessary during the time period when Respite, Behavioral Respite Services or Personal Assistance.

Wraparound Services

The goal of the Wraparound service is, quite simply, to facilitate continued involvement in Employment and Community Participation services and is, therefore, a support that enables the goals for those services. A standalone goal for this Wrapround benefit is not required. Rather, the body of the PCSP should indicate that, as needed, the person supported will access the Wraparound service in order to achieve the individualized and intended benefit consistent with the definition.
A change will impact how individuals receive services in their homes. All individuals will have access to a wraparound service that allows them to start their day at home, use their home as their base (e.g., for lunch, rest, restroom, etc.) instead of going to a facility, and return to their home after spending time at work or in the community. Additionally, the service that is currently known as “in home day” will be impacted by the proposed waiver amendments. There will still be a homebound service (referred to as a residential special need adjustment) for people who are not able to leave their homes for a period due to illness, injury, or sustained behavioral challenges. However, retirement will no longer be one of the criteria for this homebound service. Retired individuals can make an informed choice about spending time in the community and for what duration of time. They will have access to wraparound services in their homes before and after they spend time in the community.

The new services—including Exploration, Discovery, and Job Development—will help the individual figure out if he or she wants to work, what kind of job he or she might like to have and obtain the job. Individuals will also have access to ongoing job coaching services provided by their DSP staff and/or Job Coach to help them maintain their job and grow in the job over time.

Pre-Employment Services

DDA will have three different outcome-based pre-employment services: Exploration, Discovery, and Job Development. These services can help an individual and his or her family explores what working in the community would look like:

Exploration helps a person explore different jobs in the community based on a person’s interests, and existing skills. It can include business tours, job shadows and/or informational interviews with employers for people to fully understand what a job in the community could mean for them and how it could benefit them both financially and otherwise. It also includes education about work incentives for people receiving Social Security, Medicaid, food stamps, etc. It also includes information on how employment services work. After participating in

Discovery assists people who want to work in the community, but who need more information on the best kinds of job matches for them, before they pursue a job. This service helps a person identify strong interests that relate to employment, strengths and skills, and conditions for success that each person needs to be successful with employment.
